SIGNIFICANCE OF ABDOMINAL OBESITY

Abdominal obesity is known to be associated with far greater risk of comorbidities than gluteal or peripheral obesity. This difference in fat distribution has led to the description of people with abdominal obesity as 'apples' and those with gluteal obesity 'pears'; alternatively known as android or gynoid obesity, respectively. In the nineteenth century no great significance was attached to abdominal obesity. It was a relatively rare occurrence associated with well-off higher social classes, although some authors were beginning to get an inkling of its importance:
A large number of people, while of seemly proportions in other respects, grow an abdomen that is exceedingly ugly and becomes in time a great inconvenience. This is because, while the general activity of the person is considerable, their abdomen is kept free from muscular action. A protruding abdomen is patted and petted as a kind of symbol of health, when, in fact, it is sometimes, if not often, a threatening sign. It is at least a prophecy of too much fat, and as such should be looked at askance.
It is now clear that visceral obesity is of the greatest significance. Visceral obesity is adipose tissue situated within the abdomen, around the omentum of the bowel. It shares its blood supply with the omentum and secretes directly into the portal circulation. The amount of visceral fat is proportional to the waist circumference. In other words, measuring the waist not only gives an idea of subcutaneous fat around the belly but, more importantly, the amount of adipose tissue within the abdominal cavity.
