METABOLIC SYNDROME (SYNDROME X, INSULIN RESISTANCE SYNDROME)

The term 'metabolic syndrome' refers to the cluster of comorbidities, often found together in obese patients, which have insulin resistance as the common denominator. The main features are impaired glucose tolerance, hypertension, dyslipidaemia, hypercoagulability and obesity. Not surprisingly, metabolic syndrome frequently leads to type 2 diabetes, cardiovascular disease and stroke.
Metabolic syndrome was initially described in the late 1980s by Gerald Reaven, Professor of Medicine at Stanford University. Reaven (who preferred the name syndrome X, despite the mysterious connotations of the term) argued that referring to the 'metabolic syndrome' was misleading because not all the facets of the syndrome, for example hypercoagulability, are genuinely metabolic. However, the term 'insulin resistance' overlooks the fact that many of the consequences are due to the presence of too much insulin, rather than resistance to it. 'Metabolic syndrome' is currently the preferred term in most literature.
There is no connection between Syndrome X and cardiac syndrome X.
The WHO has defined the parameters of the metabolic syndrome as:
Hyperinsulinaemia or elevated fasting glucose or type 2 diabetes and at least two of the following: abdominal obesity, dyslipidaemia or hypertension.
Hyperinsulinaemia is defined as a fasting insulin level in the top quartile; impaired fasting glucose is defined as a fasting blood glucose between 5.6 and 6 mmol/L; diabetes is defined as a fasting blood glucose of at least 6.1 mmol/L or a previous clinical diagnosis of diabetes. Abdominal obesity is defined as a waist : hip ratio greater than 0.9 or BMI of 30+. Dyslipidaemia is defined as serum triglycerides of 1.7 mmol/L or more, high-density cholesterol (HDL) cholesterol of 1.04 mmol/L or less; and hypertension as 130/85 mmHg or greater.
The US National Heart Lung and Blood Institute (NHLBI) in America (ATP III) defines metabolic syndrome as any three of the following:
- Abdominal obesity (defined by waist circumference): men 102 cm, women 88 cm
- Triglycerides: >150 mg/dL (1.69 mmol/L)
- HDL: men <40 mg/dL (1.0 mmol/L), women <50 mg/dL (1.3 mmol/L)
- Blood pressure: >130/>85 mmHg
- Fasting glucose: >no mg/dL (6.1 mmol/L)
