HOW NEW CORTICOSTEROIDS NASAL SPRAYS WORK

Chronic rhinitis usually refers to chronic inflammation of the lining membrane of the nose. This results in a frequent runny, weepy, stuffy, itchy nose usually accompanied by postnasal drainage and sneezing. In addition, the inflammation creates a state of increased responsiveness, or twitchiness. This means that the nose is more easily triggered by a host of allergic and/ or non-allergic agents.
Nasal corticosteroid sprays can deter all of these symptoms and reduce the likelihood of inflammation recurring, as long as the spray is used regularly. Exactly how these agents work is not completely understood. However, what is known is that (1) they block the formation and release of histamine and other inflammation-causing chemicals from cells, (2) they reduce the number and type of inflammatory cells present in the nasal mucosa, and (3) they decrease the sensitivity of the nerves that cause sneezing. These actions go a long way toward reducing inflammation and controlling nasal symptoms.
While nasal corticosteroids are very effective in relieving nasal congestion, runny nose, sneezing, and itching of the nose, they are less effective in relieving postnasal drip and not at all effective in alleviating eye symptoms. They can, however, help improve your ability to smell. When you have an impaired sense of smell as a result of chronic rhinitis, it is because what little air does flow through your nose does not pass across the olfactory (sense of smell) area high in your nose. Nasal corticosteroids can help your sense of smell by reducing the swelling of the membrane lining your nose, thus allowing more air to flow through your nose and, specifically, more air to flow high into your nose and across the area of smell, where it is sensed by the olfactory apparatus.
Generally speaking, here is what you can expect from using these sprays:
1. Gradual improvement - not the immediate-but-temporary relief you might get from using OTC nasal decongestant sprays. Be patient. Maximum benefit from nasal corticosteroid sprays may take from one to three weeks to become apparent.
2. Regular use. In order for these sprays to work, they must be used regularly every day.
3. Twice to three times a day dosing. Most require at least twice daily use for beneficial effects.
4. Good relief of nasal congestion, runny nose, sneezing, and itching, less effective relief from postnasal drip, and no relief from eye symptoms.
5. If you have nasal polyps, your response time to these drugs may be much longer than 10 days.
6. If you have not gotten significant relief after three to four weeks of regular, twice-daily use, you should notify your physician. He or she may want to reevaluate your nasal problem and/or treatment program.
7. Once improvement occurs, reduce the dose gradually to the smallest dose required to control your symptoms.
8. Around 60 to 90 percent of people who use nasal corticosteroid sprays attain complete control of their symptoms.
