PHYSICAL EFFECTS OF OBESITY: INSULIN RESISTANCE AND OBESITY
Insulin resistance cannot cause obesity, but increasing degrees of obesity make the insulin resistance worse. In obese subjects there is more demand on the beta cell to produce insulin to maintain glucose homeostasis, and more adipocytes to undergo lipolysis. Not all sufferers from the metabolic syndrome are obese; it is possible to have hypertension, dyslipidaemia and impaired fasting blood sugar as a consequence of insulin resistance with a normal BMI, but obesity accentuates all the metabolic changes and is therefore pivotal to the concept of the syndrome.
The metabolic syndrome is thought to cause 'sympathetic hyperfunction', leading to higher blood pressure, a more active renin-angiotensin system, a raised pulse and, consequently, cardiovascular disease and even sudden death. The exact cause of the increased sympathetic activity is unclear but is thought to involve a number of compounds including insulin, leptin, cytokines, endorphins and non-esterified fatty acids (NEFAs), which are known to raise blood pressure, heart rate and increase (a-1 adrenoceptor activity, while reducing vascular compliance.
