METABOLIC SYNDROME IN OBESE PEOPLE

Not every obese person has metabolic syndrome; it is perfectly possible for an obese person to have a normal blood pressure, lipid profile, fasting blood sugar and normal insulin levels because of a normal sensitivity to insulin. However, we know from experience that many obese people have a blood pressure higher than 130/85 mmHg, with a degree of dyslipidaemia that would place them firmly in the category of metabolic syndrome. The assumption must be that a degree of insulin resistance is extremely common in the population but that only the onset of obesity causes it to be sufficiently profound to create the metabolic complications. Exactly where the line is drawn between an otherwise healthy obese individual and a sufferer from metabolic syndrome is not clear but it can be helpful to measure the fasting insulin (with the agreement of the local laboratory) because hyperinsulinism is pathognomonic of the condition. Current estimates are that 1 in 5 adults in the UK, and as many as 1 in 4 in the US, have metabolic syndrome.
Men with metabolic syndrome are more than three times more likely to die of coronary heart disease than those without it.
There are enormous ethnic differences in insulin resistance. People of non-European descent have higher insulin resistance than those of European ancestry.
